Transdermal and topical are not the same thing

Topical means on the skin. Transdermal means through the skin. A patch is designed for the second: the pad holds the extract against your skin, and Medical News Today describes them as releasing CBD slowly, with permeation enhancers added because skin is a hard barrier to cross (source: medicalnewstoday.com article on CBD patches, checked October 2026).

Most salves are plain topicals. They sit on the surface and upper layers of the skin. Some brands call a gel or cream transdermal anyway, so the word on the front is not proof of how it works.

Here is what we do not know. We cannot say how much of any given patch or salve reaches the bloodstream, and published human data on CBD patches is thin. We would rather say that than give you a number.

Wear time, dosing and the adhesive question

We read the patch page of Mary's Nutritionals (marysnutritionals.com/products/transdermal-patch, checked October 2026). Their CBD transdermal patch is a 2 x 2 inch square, 20mg, and the page says to wear it 8 to 12 hours, calls it heat, sweat and water-resistant, and says it can be cut down to a smaller dose. Other brands will differ, so read each label.

A salve has none of that structure. You decide the amount, so the dose is whatever you rub on, which is easy to change and easy to lose track of. A stick makes it a little more even, but there is no fixed milligram number per use.

Patches bring one problem salves do not. Adhesive and sweat trapped under the pad can irritate skin. Peel it off and stop using it if your skin reacts. Rotate the spot from day to day, and keep it off broken skin.

What each one costs per use

On the Mary's page we checked, the single patch listed at $7, five for $30 ($6 each) and thirty for $125 (about $4.17 each). At the time we checked, every size on that page showed as unavailable, so read those as list prices, not an offer.

A salve does not come in countable units. Our 750mg stick is $35, which is about $0.047 per mg of CBD in the container. A 20mg patch at those prices is about $0.21 to $0.35 per mg. That is not a fair race, because a patch is built to deliver and a salve may not, so read it as a check on label math, not a verdict.

How we would choose

Choose a patch if you want a fixed amount on a clock and can live with adhesive. Choose a salve if you want to decide the amount and place yourself and like a short ingredient list.

What is the difference between a CBD patch and a CBD salve?

A CBD patch is an adhesive pad you wear for hours, designed to release hemp extract slowly through the skin. A CBD salve is a wax-and-oil balm you rub on one spot and reapply when you like. The patch gives a fixed amount on a timer. The salve gives you control and no adhesive.

Are CBD patches actually transdermal?

Is a CBD salve transdermal?

Most are not. A salve is typically a topical: it stays on the skin where you put it. Some products blur the line, so check what the maker actually says about how it is formulated.

Can a CBD patch irritate skin?

Yes. Adhesive and sweat trapped under the pad can both bother skin. Use clean, dry, unbroken skin, rotate the spot, and stop if you see a reaction. A salve has no adhesive, but arnica and essential oils can irritate some people.
